Click on 'Requirements' on the left hand side of the page to expand the view.
Click on 'Forms'.
Click on 'Create Form'.
Enter the Form Name.
Click on 'Add Question' or select the question type from the list on the left hand side.
Type in your Survey Title and Description.
Type in your first question or statement.
From there you an select the type of field you want that questions to be, whether the question is required to be answered, you can duplicate a question or delete it. You can also add additional questions to the form by clicking on 'Add Question'.
The following field types are available; click on the type for more information.
- Radiogroup : Add Radio buttons as options for those filling in the form to select from. Only 1 option can be chosen from the list of options.
- Rating : Set a rating from 1 - a number of your choice where the user completing the form can rate the comment or question. Only 1 option can be chosen from the list of options.
- Checkbox : Add Checkboxes as options for the user to select from the list. The total number of boxes that can be selected from the list can be specified for each individual 'Checkbox' question.
- Dropdown : Add options to be selected from a dropdown list. Only 1 option can be chosen at the time of answer.
- Ranking : Enter multiple line items that the user can then rank starting at #1.
- Single Input : A small textbox for a short answer.
- Comment : A large textbox for longer form answers.
- Multiple Text : Multiple small text boxes for multiple small inputs. Default is 2 but additional inputs can be added.
- HTML : A large textbox where you can enter text and add HMTL formatting.
- Conditional Logic : The ability to set up logic to show certain questions based on how other questions are answered.
Once you have finished your form you can click on 'Preview' to view the form as the Contractors or Contractor Employees will see the form.
When you are happy with the form set up simply click 'Save'.
